Public Health Nurse - Best Beginnings - Healthy Babies Healthy Children
The Middlesex-London Health Unit (MLHU) has an exciting opportunity for a temporary full-time Public Health Nurse until November 2022, on the Best Beginnings team in the Healthy Start (HS) division at the Citi Plaza location, London, Ontario.
Under the direction of the Manager, Best Beginnings, the incumbent will:
· Deliver home visiting services as outlined in the Healthy Babies Healthy Children (HBHC) Program Protocol, 2018 and Guidance Document (2015)
· Conduct relevant screening and assessments with families
· Work collaboratively with Family Home Visitors within the HBHC blended home visiting model
· Provide evidence-based information and public health nursing interventions
· Refer families to health, social service, and other community partners as needed
· Work collaboratively with community agencies to ensure families receive the support and services needed, adhere to service coordination agreements with partner agencies, and provide service coordination for families as needed
· Provide breastfeeding support to families, including those experiencing significant breastfeeding challenges, and follow the Baby Friendly Initiative guidelines
· Complete accurate and timely documentation as per agency guidelines utilizing the Ministry ISCIS database and MLHU’s electronic client record
· Participate in inter-team/interdisciplinary work within the Healthy Start Division
· Perform other duties as assigned.
QUALIFICATIONS:
· Baccalaureate Degree from an accredited nursing program (BScN)
· Current certificate of competence from the Ontario College of Nurses (CNO)
· Proficient technical skills in the areas of documentation, Microsoft Office, and database software
· Ability to work flexible hours
· Valid Ontario Driver’s license and own transportation
· Excellent interpersonal and written/oral communication skills
· Demonstrated experience and expertise in critical thinking, problem-solving, and relationship-building
· Ability to work autonomously in a variety of home and community settings
· Demonstrated skills and experience in evidence-informed decision-making and leadership
· Demonstrated knowledge, skills and experience with home visiting pregnant individuals and/or families who are at risk and/or experiencing challenges
· Knowledge and experience in providing breastfeeding support, as well as assessing newborns and postpartum mothers
· Experience and expertise in working with priority populations and demonstrated ability to create safe and inclusive environments when working with diverse populations
· Demonstrated understanding of health equity and experience in taking action to address health inequities
· Training in NCAST, BFI and PIPE is considered an asset
· Communication proficiency (written and verbal) in Arabic, French, Spanish and/or any other language(s) is highly desirable.
